2. If you are ordering Remel ATCC products, you must sign a Remel ATCC Licensed Derivative Program End User Agreement. I have attached the link below.
This agreement is based upon Ship To addresses. So if there is more than one person/department ordering these product(s) on your campus you will have to fill out an End User Agreement each time unless the product is always being shipped to the same ShipTo Address (i.e., Madison campus always ships to their MDS dock so they will only have to sign one if anyone orders a Remel ATCC product).
Please sign this agreement at time of order and fax to Mark Hohlstein for his files so your order can be placed with no delay.
