Death of Employee

If you are the family member of a UW employee who has recently passed away, you should contact the employee’s benefits office immediately for information about how to continue benefits and apply for any death benefits available.

State Group Health Insurance

If family coverage was in force at time of death, coverage for insured family members continues automatically at the full premium rate (no employer contribution). Coverage for the spouse or domestic partner may continue for life and coverage for children may continue until they no longer meet the eligibility requirements of the plan.

If family coverage was in force at the time of death, any unused sick leave credits can be used to pay the monthly premium, or the credits can be escrowed (saved) for future use. See the Sick Leave Conversion Credit brochure for details.

Surviving family members may voluntarily terminate coverage by providing written notification to the Department of Employee Trust Funds (ETF). Coverage will end on the last day of the month in which ETF receives written notification to end coverage.

If single coverage was in force at the time of death, any premiums paid for coverage months following the date of death will be refunded.

EPIC Benefits+

EPIC Benefits+ includes an accidental death benefit. If the death was accidental, submit an Accidental Death Claim form to EPIC Life Insurance Company.

If family members were covered at the time of the employee’s death, coverage continues to the end of the month for which any premiums have been paid.  Family members have 60 days from the end of the month in which the death occurred to submit a continuation form and application to EPIC Life Insurance Company to continue coverage. 

Coverage for a spouse or domestic partner may continue for life and coverage for children may continue until they no longer meet the eligibility requirements of the plan.

If single coverage was in force at the time of death, any premiums paid for coverage months following the date of death will be refunded.

Dental Wisconsin

If family members were covered at the time of the employee’s death, coverage continues to the end of the month for which any premiums have been paid.  Family members have 60 days from the end of the month in which the death occurred to submit a continuation form and application to EPIC Life Insurance Company to continue coverage. 

Coverage for a spouse or domestic partner and eligible children may continue for 36 months.

If single coverage was in force at the time of death, any premiums paid for coverage months following the date of death will be refunded.

VSP Vision Insurance

If family members were covered at the time of the employee’s death, they have 60 days from the end of the month in which the death occurred to submit a continuation form and application to VSP to continue coverage.

Coverage for a spouse or domestic partner and eligible children may continue for 36 months.

If single coverage was in force at the time of death, any premiums paid for coverage months following the date of death will be refunded.

State Group Life Insurance

Survivors should submit a Death Benefit Information Request form to the Department of Employee Trust Funds (ETF) to report the death. Survivors may also call ETF at 1-877-533-5020 to report the death. ETF will notify Minnesota Life, who will send a claim form to the beneficiary.

If the employee carried Spouse/Domestic Partner/Dependent coverage, coverage ends 90 days after the date of death (no premium due during this period).

Surviving family members have 90 days from the date of death to submit an application to convert Spouse/Domestic Partner/Dependent coverage to an individual policy (each family member must convert coverage separately).

Any premiums paid for coverage months following the date of death will be refunded.

Individual and Family Life Insurance

Survivors should contact the applicable UW benefits office to report the death. Minnesota Life will send a claim form to the beneficiary.

Surviving family members are eligible to convert coverage to an individual policy (each family member must convert coverage separately).

UW Employee’s Inc. Life Insurance

Survivors should contact the applicable UW benefits office to report the death. Minnesota Life will send a claim form to the beneficiary.

Any premiums paid for coverage months following the date of death will be refunded.

University Insurance Association (UIA) Life Insurance

Survivors should contact the applicable UW benefits office to report the death. Minnesota Life will send a claim form to the beneficiary.

Accidental Death and Dismemberment

If death was accidental, call the Zurich American Insurance Company at 1-866-841-4771 to report the death and request a claim form. Zurich must receive the claim form within 90 days of death.

If family coverage was in force at the time of the employee’s death, covered family members have 60 days from the date of death to convert coverage to an individual policy (each family member must convert coverage separately). You may convert coverage by contacting Zurich American Insurance Company at 1-800-834-1959 (reference UW System Policy # GTU-8364005).

Any premiums paid for coverage months following the date of death will be refunded.

Income Continuation Insurance

Coverage ends upon employee’s death.

Any premiums paid for coverage months following the date of death will be refunded.

Employee Reimbursement Account (ERA)

Money remaining in the employee’s Health Care or Dependent Day Care FSA account is accessible after death for expenses incurred by the employee or eligible dependents prior to the date of death. Surviving family members must submit a claim form and proof of death to WageWorks to receive reimbursement. Go to WageWorks or contact WageWorks at 1-855-428-0446 to start the claims process.

Tax-Sheltered Annuity 403(b) Program

Survivors should contact TSA investment company for information regarding benefits payable upon death.

Wisconsin Deferred Compensation 457 Program

Survivors should contact Wisconsin Deferred Compensation at 1-877-457-9327 for information regarding benefits payable upon death.

Wisconsin Retirement System

Survivors should submit a Death Benefit Information Request form to the Department of Employee Trust Funds (ETF) to report the death. Survivors may also call ETF at 1-877-533-5020 to report the death.

ETF will send a claim form to the beneficiary.
