APPENDIX H - WISCONSIN SALES AND USE TAX EXEMPTION CERTIFICATE

This is to certify that _____________________ is a University of Wisconsin employe traveling on university business and is therefore exempt from payment of Wisconsin Sales and Use taxes, including local tax, on meals, lodging and car rentals under s. 77.54(9a), Wisconsin Statutes. The University of Wisconsin's tax exemption number is ES 40706.
Please include the name of the University of Wisconsin with the billing/invoice and retain a copy of this memo to substantiate that the sale was tax exempt.
