Controlled substances

To dispose of a registered controlled substance, the registrant must complete DEA Form 41. The registered controlled substance must remain in possession of the registrant until retrieved by the waste contractor. Notify the contractor you have a registered controlled substance when you request a pickup.
Legacy controlled substance disposal
Legacy controlled substances are substances that are listed on one of the US Drug Enforcement Agency's (DEA) Controlled Substance Schedules, but are unregistered. This can result from a variety of circumstances. Possession or use of controlled substances without a registration is illegal. Disposal is difficult and subject to approval by DEA.
Obtaining Authorization to Possess Controlled Substances
In accordance with federal and state laws, it may be possible to obtain a Special Use Authorization (SUA) to possess controlled substances for research, analytical laboratory, industrial/commercial processing, instructional and other activities
You will need approval from both the State of Wisconsin Department of Safety and Professional Services and the U.S. Drug Enforcement Administration (DEA).

Note no fee is charged for permits issued to employees of state agencies or institutions.

You may file forms simultaneously. If you do so, write "Pending" in the State Controlled Substance License Number space in section 4 of the federal form.
